Where to Start

Planning can feel overwhelming, but you’re not alone. Cremation Services Only and our trained staff are here to support you and help you through your grief journey while honoring your loved one. To help you get started, below is some helpful information. Please feel free to contact us with any questions.

 


 

First Steps

  • If your loved one dies at home or somewhere that isn’t a healthcare facility, you’ll need to contact emergency personnel before they can be taken into our care.
  • If your loved one was under medical care, you should call their physician as well.
  • If your loved one dies in a healthcare facility, they will notify the proper authorities, including your chosen funeral home or cremation service if they have that information.
  • When you are ready for our services, simply call us. We are available 24/7.
  • We will need your loved one’s full name and the location we will be picking them up. We also require payment over the phone, via credit card before sending out one of our transfer specialists to your location.
  • If your loved one hasn’t passed yet, but you would like to make arrangements, please call us and we can help you prepare for when the time comes.



Next Steps

  • Once your loved one is in our care, we will provide you with forms that will give us the necessary information for the death certificate and authorize the cremation to take place.
  • Required Forms:
    • Cremation Authorization
      The next of kin will need to sign the cremation authorization form. If your loved one has a spouse, they would be the next of kin. If not, the majority of children’s signatures will be required. If your loved one was not married and does not have children, we can help you to determine who would legally be considered the next of kin. Cremation is not able to take place without signatures from the next of kin.
    • Registration and Vital Statistics
      This form will give us all the information we need to complete and file the death certificate. Death certificates will be filed in the county of where your loved one passed.
  • We will obtain all the necessary permits needed for the cremation process to take place.
  • Once we have all of the proper authorizations, permits, and information and cremation has taken place, we will mail your loved ones cremated remains directly to you.

When you call us to begin planning final arrangements, we can give you a quote over the phone. Even if your loved one hasn’t passed yet, we can mail, fax, or email you the necessary paperwork, which can be completed beforehand and on file for when the time comes.

We specialize in simple cremation services and want to help you through a time that can feel complicated. With our direct cremation service, there are only a few small decisions that need to be made and our directors will walk you through this process.

What we do:

  • Provide transportation for your loved one from the place of death to the crematory
  • Obtain necessary county permits for the cremation process to take place
  • Prepare and file the official death certificate and notify social security
  • Provide the required cremation container and minimum temporary urn for cremated remains
  • Complete the cremation process
  • Ship your loved ones cremated remains to you

All you have to do:

  • Provide vital information about your loved one for the death certificate
  • Complete a cremation authorization form, signed by the next of kin
  • Providing an address for where the cremated remains will be sent once the cremation process is completed

  • Social security number
  • Place of birth
  • Father's name
  • Mother's maiden name
  • Highest education
  • Occupation

  • Informants name and address
